ALL PAPERWORK IS FOUND TO THE LEFT UNDER "QUICKLINKS"

The RDT, Emergency Contact, and Health History Update forms can be handed to Ms. Bogdewicz or Ms. Toth. They can also be uploaded via the Genesis Parent Portal. The Physical Paperwork must be handed in in person.

Athletes must have a current sports-qualifying physical exam on file in the Nurses Office.  

CURRENT PHYSICALS MUST BE TURNED IN TO THE ATHLETIC OFFICE OR NURSE PRIOR TO PARTICIPATION
